RYAN WHITE CARE SERVICES
GBAPP has expanded its activities and is the fiduciary, contractor, and lead agency for the Bridgeport Primary Care Consortium for Ryan White Title I funding. In this role, GBAPP subcontracts with five local health and social services agencies in Bridgeport to provide a seamless network of care to persons of all ages living with HIV/AIDS. GBAPP’s partners in the Bridgeport Primary Care Consortium are Optimus Health Care, Inc., Life Bridge Community Services, Liberation Programs, Inc., Regional Network of Programs and Southwest Community Health Center.

The Women’s and Men’s Health Project (WMHP) provides a continuum of care for marginalized women, men, and adolescents infected with HIV/AIDS. Case managers seek out women, men, and adolescents in the community and offer them a variety of services such as HIV/AIDS testing, counseling, treatment, primary health care, mental health services, substance abuse treatment, and case management. The WMHP office is also located at 1470 Barnum Avenue, Bridgeport, CT. GBAPP is also the fiduciary, contractor, and lead agency for the Bridgeport Primary Care Consortium for Ryan White Title I funding. OUTREACH, TESTING, LINKAGE
1) Conduct community and online outreach in order to engage members of the priority population (heterosexual, African-American males ages 18+), disseminate HIV prevention information, raise HIV/AIDS awareness, and promote and offer HIV testing.
2) For those who are infected with HIV, establish a linkage into HIV medical care systems.
3) Make appropriate referrals to medical, social, and prevention services.
4) Integrate targeted condom distribution activities into the program.
5) Link patients to PrEP, including navigating insurance and other payment systems, linkage to a PrEP medical provider, and support with obtaining and adhering to PrEP. This program is funded by the State of CT Department of Health AIDS Program.
GBAPP serves as the fiduciary and also provides leadership and infrastructure to the Bridgeport United Coalition, a Drug-Free Communities funded initiative that advises local substance-abuse, youth prevention programs. This program is funded by the Center for Disease, Prevention and Control.
